Establishing Care
To establish care is one-time fee of $550.
This includes an initial, comprehensive lab panel and a one-hour appointment to review your results and develop your care plan.
For Primary Care or BHRT patients, this one-time fee also covers your first-month’s membership fee.
For Weight Loss & Regenerative Medicine patients, this one-time fee also covers your first two-month’s membership fee.
For Functional Medicine patients, this one-time fee includes a specialty lab review. Please note that additional specialty testing fees are not covered, and will be billed separately.

Submit a new client form above & our team will reach out to you to schedule your initial visit. We'll run your lab panel, then meet for an hour to review your results and build a plan around your goals.

Choose Your Care Plan
Select the membership that fits you: Primary Care, BHRT, Functional Medicine, or Weight Loss & Regenerative Medicine.
Step Three
Ongoing, Personalized Care
Routine labs, follow-up visits, and adjustments keep your plan aligned with your progress over time.

Absolutely. Your needs may evolve, and our plans are designed to adjust accordingly—whether that means refining goals, adding resources, or restructuring timelines.
What payment methods do you accept?
For simplicity and clear documentation, our engagements are billed via invoice. You will receive a detailed statement outlining services and timelines for payment.
