Preventive care, prescription management, and treatment for everyday illnesses and ongoing conditions. Longer visits and a lasting relationship with your provider give you care that reflects your history, priorities, and long-term health.

  • Includes: Prescription Management, Quarterly Labs with Review, and Access to B12 and Glutathione Injections / $15 per injection.

Bioidentical Hormone Replacement Therapy (BHRT)

Evaluation and treatment for hormonal changes that may affect energy, sleep, mood, and sexual health. When appropriate, bioidentical hormone therapy is tailored to your symptoms, lab results, and medical history, with ongoing monitoring and adjustments.

  • Includes: Primary Care, Hormone Prescription Management and Medication Costs, Quarterly Labs with Review, and access to B12 and Glutathione Injections / $15 per injection.

Functional Medicine

Personalized care for concerns such as digestive issues, persistent fatigue, food sensitivities, and brain fog. We take a closer look at your health history, nutrition, lifestyle, and environment, using targeted testing when appropriate to identify contributing factors and guide treatment.

  • Patient Pays for Specialty Labs

  • $125 per subsequent 30-minute follow up appointments

Weight & Regenerative Medicine

Medical weight management with nutrition guidance, body composition tracking, and prescription treatment when appropriate. Care also includes evaluation for select peptide and NAD+ therapies, such as sermorelin, based on your medical history and treatment goals.

  • Includes: Monthly InBody Body Composition Measurements and Vital Signs, plus two patient portal or text check-ins per month.

  • Please Note: Weight Management and Regenerative Medicine Medications are billed separately.

Aesthetic Treatments

  • Yes! Linden can serve as your primary care provider, including routine and preventive care, sick visits, medication management, and ongoing health needs.


  • Depending on your needs and treatment preferences, options may include bioidentical hormone pellets, topical creams, oral progesterone, and testosterone injections. Treatment and dosing are individualized based on your symptoms, health history, and lab results.

  • Not always! Membership is required for prescription and medication management services.

    Functional medicine visits are offered on a pay-per-appointment basis, and aesthetic services do not require a membership. An initial lab panel is required for medical management services.

  • Functional medicine looks at how different aspects of your health may be connected rather than addressing each symptom in isolation. At Linden, this may include concerns related to gut health and digestive symptoms, food sensitivities, chronic fatigue, brain fog, inflammation, and environmental exposures. Specialized testing may be used when appropriate to identify contributing factors and guide a more targeted approach to care.
